Schedule Tribes (ST) Certificate

How to Apply for Schedule Tribe (ST) Certificate.

1. Obtain an application form for ST Certificate from Jan-Suvidha Centre,DC Office ,Hawai with minimal processing fee.
2. Fill in the form with all necessary details required.
3. Recommendation of verifying authority (For rural areas GB/HGB/ASM should recommend) is required before submission.
4. Submit the filled in application form along with the necessary supporting documents to Jan-Suvidha Centre.

Supporting Documents Required.

1. Father’s ST Certificate or Birth Certificate of person required the certificate.
2. Aadhaar Card.
3. Two Nos of recent passport size photo.
